Change the name of your submit button to "submit" not "Submit". Also to test your php code I need it as an email attachment not embedded in an email because email handling systems can change the coding (ie they change some chartacters). So there may or may not be an issue here. If you
have only edited it in the Form Builder or a text editor it sould be fine.
Thanks for your feedback. We have deliberately minimized the complexity of the model and as you mention only focus on physical assets for depreciation. Intangible items ie intellectual property, brand, goodwill etc can actually be envisaged as part of the Other component of the valuation. This does assume that the value of intangible assets is maintain through the operations of the business and as such the depreciation component of this is by default covered by the business operating costs. For example development staff continue to develop intellectual property therefore the cost of this staff is in fact the depreciation value of the ip (the cost to maintain/replace it). You can also apply this concept to brand ie the marketing expenses are the cost of maintaining the brand. There are many other components which contribute to these however they may serve as a base example.

I am not sure which software title you are referring to but if it is an Excel based title generally all input is in on the input sheet and in the blue cells (blue cells are unprotected). All non input cells are protected to ensure formula integrity. Macros must be enabled for the input sheet to be accessed.
Check for duplicates can be done using Goldmines dupcheck function. You can do this automatically using the attached script (WebImp4.php). I have also attached a sample form to show the form setup. Instructions are provided within the files (open them in a text editor or view source to read).
The registration data is held in a cookie on your system. If cookies are not enabled they cannot be saved or possibly they are being cleared/removed by some software? One of these may be your problem?
Give our test form (http://www.softswot.com/Form1Try.php) a run to see the format of delivered data. If you are talking about the web form layout you can use any valid form. Our form software is availabe for free evaluation, you can give it a test run to see if it meets your needs.
Macros are simply part of the Excel operating system. If you are restricted from using them the software will not function. If you like I can email you the zipped file (which can be downloaded) but you will still need to enable macros to run the software.
Use the command buttons to added lines/categories. The worksheet is protected to maintain formula integrity, full functionality is provided via the command buttons.

You can simply copy the form code (everything between the form tags including the tags) and paste it where ever you want it in your exising web page file using a text editor (it must be within the body tags fo your web page plus copy the php processing code (at the top of the built form) and paste it to the top of your web page code above everything else.
You can download and review our Shift Scheduler from:http://www.bizpeponline.com/ShiftScheduler.html. It covers most of your requirments using the constraint settings and the manual input to set a must work/must not work requirements. If the basic function is of interest but you would like to incorporate extra features we can usually accomadte these with additional development work.
A single user license allows you to use the software on any site (domain) owned by the licensed user. So if you own 10 domains you can use the software on all 10. Additional licenses are rquired to use the software on sites owned by others.

I am not entirely clear on your complete requirements but our Shift Scheduler may provide some support. Try setting each of the 50 locations as a shift with its own staff requirement. You can set the start time and length the same for each location or allocate a portion of the total shift to each location ie location A is 8:00 to 8:30 location B is 8:30 to 9:00 etc (shifts should be arranged in chronological order). For a complete day this would result in 150 shifts ie 50 locations for each of your 3 time blocks (shifts). It may be best to have a play with a small number of locations to see if the result is along the lines required before you try a full scale run. If the basic result structure is along the lines required the software can be refined to suit ie adjust shift names etc. This would be a reasonably economical solution to your problem.

Forms built with unregistered software will expire they are only for evaluation. Once you register you can rebuild your form with the registered software.

On the accuracy: The indicators are compounding (as they should be) not additive so 100*105%*105%=110.25. In other words a 5% increase on a 5% increase results in an overall increase of 10.25%.
On the protection: All non-input cells and Visual Basic code are protected to ensure integrity and protect source code in Non-Commercial licensed software
We do offer Commercial licenses for the use of software for inter-business support, advice, or consulting and upon request with a Commercial license can provide an unprotected software version allowing you to view the formula applied to enhance your knowledge of software operation (macros remain protected).
We also have Source licenses where everything is accessiable. The most suitable license depends upon your requirements.
It sounds like you have a formatting inconsistancy in your PL file. Check the help notes on Format Requirements against the PL file you import for inconsistancies.

A single license is suitable for any domain (site) owned by the licensed
user. Form1 will run on both sites as long as they both support php. My guess is you may have set some of the variables incorrectly or made changes that have broken the php code?

The form data is sent as a text email. You can covert this to a csv data file on reciept if required. With developemnt work it would be possible to generate a csv file on the fly and send this with the email.
Does your server support php? Check it at http://www.softswot.com/phptest.php If so php is the easiest to get running. Move it out of the cgi-bin and place it on your server just like a stantard html file. Only the pl script needs to be in the cgi-bin if you are using it. At the moment you have different cases in the file on the server name and the file name you call, ie your uploaded file is webimp2.php and you call WebImp2.php. So when you move the script file change the name back to WebImp2.php and adjust your call action url to the new location. Also you currently have name="redirectURL" value="clearsightdownload.htm" use the full url http://www.davincitools.com/clearsightdownload.htm. At the monent you are trying to find this url in the cgi-bin and it is not there.
